Neonatal bronchopulmonary dysplasia: morphogenesis, morphological diagnosis

Four groups of changes (premorbid, paradysplastic, nonspecific and pathognomonic) have been detected in the lung of 64 dead children with neonatal bronchopulmonary dysplasia on artificial ventilation for an average of 20±0.1 days. In the latter group, the authors have identified the key structures that reflect the pathogenetic action of aggressive factors of AV and that are diagnostic markers of the 4 detected stages NPBD morphogenesis: bronchiolectasis and stellar atelectasis (Stage 1); rosette-like bronchioli (Stage 2), mosaic pneumosclerosis and fibroatelectases (stellate, moire and fibromascular) (Stage 3); and honeycomb (Stage 4).

Cellular renewal in the lung and morphological manifestatoins of bronchopulmonary dysplasia in neonates

The lungs of 184 dead neonates and babies who were from birth on artificial ventilation with rough parameters and hyperoxygenation were found to have peculiar pathohistological changes that were used to work out an illustrated algorithm for the morphological diagnosis of neonatal bronchopulmonary dysplasia (NPBD). An immunohistochemical study was used to identify the cell renewal markers Ki67, bcl2, p53 and CPP32. In NBPD, there was an extremely high expression of all the markers in the cells of the bronchial, alveolar epithelium and lung interstitium as compared with the control group. It is concluded that cell renewal is accelerated In the lung in NPBD and the disease progresses until death occurs in infants.

The influence of spa-and-resort factors on the general well-being of practically healthy people

Under the influence of complex spa-and-resort factors of the sanatorium "Ural" in practically healthy people the level of anxiety did not change, the indicators of well-being significantly increased. The state of immune system changed: the number of CD8+ lymphocytes reduced: the concentration of IgG, phagocytic activity and hemolytic activity of complement system increased.The data can be used to develop of impact regimes of spa-and-resort factors in practically healthy people in order to obtain the most pronounced health effect.

Bestim is a synthetic dipetid – γ-D-Glu-L-Trp – gamma D-glutamyl-L-tryptophan sodium salt. The drug was developed at State Scientific-Research Institute of Highly Pure Biochemicals, St. Petersburg and allowed for clinical use in the treatment of patients with secondary immunodeficiency by the Russian Ministry of Health. The data about the ability of Besim to decrease the activity of Th2-and to increase Th 1 - an immune response by reducing the production of IL-4 and the stimulation of IFNy are present. Bestim also reduces the secretion of IL-8 and increases the production of IL-10. Application Bestim accompanied by positive shifts of immunological parameters and clinical symptoms of chronic urogenital chlamydia infection, peptic ulcer disease, secondary syphilis, skin and mucous membranes, chronic recurrent vulvovaginal candidiasis. Appointment of Bestim also increases the effectiveness of vaccination against hepatitis B in patients with secondary immunodeficiency.

The adaptation potential of student youth

The research data concerning the social psychological adaptation of student youth to the general social conditions and the immediate vitality environment is discussed. The University entrance is characterized by changing of activities and hence intensifying of the adaptation processes. The students experience higher psychological physiological stress factually of all body systems. It results in the development of chronic non-infectious diseases among future specialists. On the basis on the analysis of positive-negative assessment by the students of major aspects of their life and the level of being satisfied with occurring social changes it was established that only 28,1% of students managed to fit into the new social environment and to adapt to the conditions of modern life.

Analysis of fetal hemoglobin level in newborn infants with multiple organ dysfunction syndrome

Multiple organ dysfunction syndrome (MODS) is the most frequent cause of death of newborns in neonatal intensive care units. Level fetal hemoglobin (HbF) in dynamics at 107 newborns with MODS is studied. Criteria of inclusion: insufficiency of two and more systems at the newborn in early neonatal period. Criteria of an exception: chromosomal anomalies, blood transfusion in the first 15 days of a life. Definition HbF was spent on 3, 7, 11, 15 days of a life on hemoximetr ABL 800 Flex Radiometer Copenhagen (Denmark). Newborns with the maximum estimation on scale NEOMOD of 5 and more points (n=52) have made the basic group, newborns with an estimation of 4 and less points (n=55) have made comparison group. Groups of newborns are comparable on a sex, gestational age (p> 0,3). Level HbF at newborns of the basic group on 3, 7, 11, has made 15 days of a life 92 (82-94), 94 (78-98), 94 (79-99), 91 (75-97), at newborns of group of comparison 89 (79-94), 87 (77-93), 85 (73-93), 80 (70-90) % from the general Hb, accordingly, (Me, LQ-UQ). At comparison of level HbF in groups on 7, 11, 15 days of a life are received significant distinctions (р <0,01), distinctions for 3 days are not received (р=0,62), the test the Mann- Whitney. The conducted research has shown that at newborns with MODS takes place reactivation of HbF synthesis HbF. Possibly, in conditions hypoperfusion at MODS hemoglobin with high affinity to oxygen provides more effective gas exchange.

Clinical value of erythrocyte indices in premature newborn infants with multiple organ dysfunction syndrome

Red blood cell indices become more significant in clinical practice, considering high level of accuracy of measurement which is provided with the automated devices. The purpose of the present research – to define clinical importance of red blood cell indices at preterm newborns with multiple organ dysfunction syndrome (MODS). In research it is included 112 preterm newborns with MODS. Definition MCV, MCH, MCHC, RDW was spent for 3, 7, 11, 15, 20, 28 days of a life. At preterm newborns with heavy displays MODS in early neonatal period pressure erythropoiesis takes place, on what specifies increase MCV and MCH. Decrease MCV and MCH by the end neonatal period at newborns with heavy displays MODS testify to possible deficiency of iron. Studying of sensitivity and specificity RDW as additional criterion of an estimation of weight of a condition and forecast MODS at newborns is a perspective direction in neonatology.

Functional status of red blood cells in newborns with severe respiratory distress syndrome (short message)

In newborns with severe respiratory distress syndrome regardless of gestational age at improving the state of increases glucose uptake erythrocytes. Acid resistance of erythrocytes is reduced, the population of red blood cells becomes more homogeneous, as indicated by the decrease in total time of hemolysis, a shift stage at a maximum rate of hemolysis to the left (the stage at a maximum rate of hemolysis is earlier), reducing the number of high-resistant red blood cells, increasing the maximum level of hemolysis. There are some differences in acid resistance of erythrocytes depending on gestational age and sex.

The investigation of dynamic changes of erythropoietin content of the erythroblast island cultures

Dynamic changes of erythropoietin content on the 6th, 12th, 24 and 48th hours of cultivation in bone marrow erythropoietic island liquid short-term cultures were investigated. The initial doses of 0.25, 0.5, 1 mE/ml of erythropoietin was used, and one series with no erythropoietin as control. In culture with 0.25 mE/ml of erythropoietin, its reduction was registered from the 6th to 48th hours of cultivation with relatively lower number of erythroblast islands in state of involution, and a higher number in the state of reconstruction. In cultures with 0.5 mE/ml of erythropoietin, the same erythropoietin reduction was registered with relatively higher numbers of erythroblast islands m the state of reconstruction, and with an unexpected elevation of erythropoietin concentration on the 48th hour of cultivation. In cultures with 1 mE/ml of erythropoietin, such unexpected rise in erythropoietin content was registered in 24 hours, accompanied by relatively higher number of erythroblast islands in the state of reconstruction.

To investigate a potential role of calcium and water channels (aquaporins) in cataractogenesis, the lens transparency was analyzed in vitro. Influence of ceruloplasmin on red blood cells count and resistance under severe physical exertion

Severe physical exertion results in the red blood cells osmotic fragility decrease, erythrocyte lysis starting period reduction, cytomembrane sorption capacity increase. Ceruloplasmin insertion 24 hours before severe physical exertion helps to normalize red blood cells membrane resistance.
