Cytokines in neoplastic processes of the cervix associated with human papillomavirus high carcinogenic risk

Human papillomavirus high carcinogenic risk belongs to the initiating role in the development  of cervical intraepithelial neoplasia and cervical cancer. Virus infection does not always cause pathology. Development of the pathology is determined by the state immunity and, primarily, local  protection of the reproductive tract of women. Central role in the regulation of immune responses  directed against infectious and neoplastic processes play cytokines. Objective: To study the levels of cytokines (IFN-α, IFN-γ, IL-1β, IL-2, IL-4, IL-8, IL-10, TNF-α) of cervical mucus in patients with CIN I, CIN II and CIN III, associated with human papillomavirus high carcinogenic risk. The study established significant changes of cytokine profile in neoplasia of varying severity, which  have both general regularities and distinctive features associated with the progression of the pathological process of the cervix.

Description of cytokine status in persistence HPV infection

The persistence of human papillomavirus high carcinogenic risk took place against a background of reduced immune reactivity is a major risk factor for cervical neoplasia. The levels of IL-1 β, RaIL 1, IL-2, IL-4, IL-8, IL-10, IFN γ, IFN α, TNF α has been studied in the cervical mucus of women with persistent and transient course of human papillomavirus infection. The results allow to conclude multi-directional changes in the levels of cytokines that determine the development of Th 1 immune response (decrease in the concentration of IL-2 and increased IFN γ), regardless of the persistence or elimination of HPV. At the same time, women with persistent had increase of IL-4, which determines the development of Th 2 immune response.

The subpopulation structure of lymphocytes of a peripheral blood at patients with cervical intraepithelial neoplasias, associated with a HPV high cancerogenic risk

After immunofenotypification of lymphocytes by the method of flowing cytometry at patients with CIN I, II and III degrees have been established following features: unidirectional changes of the maintenance of cells with phenotype CD3+CD4 + and CD3+19 + at the development CIN I-II; the distinctive features at CIN I - depression CD3+19- and CD3+8 + lymphocytes, at CIN III - falling CD3+HLA-DR + and CD3+16+56 + cells in peripheral blood. The progression of a cervical intraepithelial neoplasia, associated with HPV-infection is accompanied by oppression effectual and regulatory functions of T- lymphocytes, which supports pathological process and raises probability of a malignancy.

Effect of immunocorrection on surgical outcomes in patients with autoimmune-aetiology vasculitis and vasculopathies

The present study was undertaken to investigate and compare the outcomes of three options of preoperative prepara­tion and postoperative management of patients presenting with autoimmune-aetiology vasculitis and vasculopathies, who had been subjected to angiosurgical interventions over a 7-year period. According to the aetiopathogenetic sign, the majority of the patients appeared to have thromboangiitis obliterans, non­specific aortoarteritis, primary and secondary vasculopathies. All the patients were age- and sex-matched and well comparable by the underlying disease, the level of lesions of the vascular bed, and the ischaemia burden. The study was performed retrospectively. A total of three options of preoperative preparation and management of the postoperative period were used: option one — treatment comprising plasmapheresis, immunoglobulins and biological immunomodulators; option two — using cytostatic agents and hormone therapy; and option three — consisting of conventional basic vascular therapy alone. The comparative analysis performed demonstrated a clear advantage ofimmuno-correction and efferent techniques of treating patients presenting with the pathology concerned. The use of the therapeutic-and-diagnostic algorithm we are suggesting herein would make it possible to avoid complications in the immediate postoperative period, to reduce the duration of the patients' hospital stay averagely by l0 days, andtoprolong persistence of the obtained positive results by 11.5%

Effects of short-term immunosuppression on the engraftment of skin transplants at syndrome of the diabetic foot

Effects of a single administration of small doses of 5-fluorouracil on results of skin plasty for syndrome of the diabetic foot were studied in 61 patients who had undergone local operations for pyo-necrotic diseases of the foot against the background of diabetes mellitus. The completion phase of their treatment was free dermal plasty with a split skin graft. Administration of 5-fluorouracil during the plasty operation was found to be accompanied by less marked inflammatory reaction in the transplant bed and in the graft. The frequency and square surface of lysis in the transplanted dermal skin graft were considerably lowered.

Application of new technologies in resection of the liver

Against the background of operations using expensive equipment, the results of liver resection by the apparatus-plastic method produce favorable impression. Positive results are obtained due to the simplicity of performing resections and reliable sealing of the wound surface by the laser "welding". Favorable course of the postoperative period depended on the minimal volume of the intraoperative blood loss, the absence of exude complications in the postoperative period.
